Job Description

Heritage Management Services is currently seeking a highly qualified Division MDS Coordinator (RN) to oversee timely completion and transmission of resident assessments, as well as organize and develop the overall operation of the nursing departments of two of our non-profit facilities in accordance with state and federal guidelines.

Duties & Responsibilities:

  • Schedule residents for assessment per federal and state requirements
  • Develop, complete and transmit an accurate MDS for residents within specified schedules and federal and state requirements
  • Participate in Interdisciplinary Care Plan Meetings to coordinate care needs identified in the MDS
  • Assist the Director of Nursing in Planning, developing, organizing, implementing, evaluating, and directing the nursing service department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern nursing care facilities
  • Monitoring of resident care and services to achieve quality outcomes and high customer satisfaction

Qualifications & Requirements:

  • Current Nursing licensure as an RN
  • Detail-oriented
  • Strong Organizational and Assessment Skills
  • Experience in skilled nursing/or sub-acute care setting
  • Two years of experience with MDS development, completion and timely transmission or similar process in another health care setting
  • Critical thinking and problem solving abilities
  • Strong computer skills
  • Ability to meet regulatory deadlines
  • Enjoys a brisk and energetic work pace
