Job Description

Sierra Home Health & Hospice is currently seeking a Registered Nurse (RN) Case Manager & Liaison with excellent communication skills to service and grow our patient population in Las Cruces. The ideal candidate will have excellent interpersonal skills and case management experience to facilitate physician and patient communications. 

Duties & Responsibilities:

  • Develop rapport with physicians, office staff, and other referral sources in order to increase census
  • Build trusting relationships with patients and family members
  • Evaluate and treat patients using the most current technology and practices
  • Develop initial plan of care based on communication with the physician and patient assessment
  • Participate in superior clinical outcomes monitoring, follow up, and agency performance improvement initiatives
  • Manage and educate Home Health Aides and LPNs performance in implementing nursing services
  • Collaborate with an interdisciplinary team to provide highest level of care

Qualifications & Requirements:

  • Current and unencumbered license to practice as a Registered Nurse in the state of New Mexico
  • Minimum of one (1) year of home health nursing experience
  • Current CPR certification
  • Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years
  • Exceptional customer care
  • Excellent written and verbal communication skills
  • MS Office skills (Word, Excel, PowerPoint)
