POSITION: Director of Social Services


The primary purpose of the job position is to plan, organize, develop, and direct the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.


• BSW or related degree in behavioral sciences

• Previous experience as a Director of Social Services in a skilled nursing facility is preferred

• Must have a proven track record of integrity and performance, and knowledge of the Long Term Care industry (customer satisfaction, regulatory compliance)

• Embrace the standards, values, and beliefs of the organization

• Excellent communication skills

• Excellent organizational skills

• Must exhibit sound judgment making skills, patience and compassion

• Be capable of maintaining effective working relationships with residents, family members, staff members and stakeholders

• Possess the flexibility to approach skilled nursing and long term care services through a hospitality based model

• Bilingual is preferred but not required


• Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility

• Development and implementation of resident assessments and plan of care

• Assist residents and family members with transition to facility

• Participate in discharge planning

• Provide assistance with referrals for services and equipment

• Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident

• Assist resident/families in completion of Medicaid application and assist resident/family in scheduling appointment with local Income Support Division.

• Follow-up with local Income Support Division as directed, who are Medicaid pending until approval is received

• Maintain a quality working relationship with the medical profession and other health related facilities and organizations

• Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and challenges of the resident through the development of social service programs and referrals to appropriate agencies
