(Needs to have nursing background) (LVN/RN)
The primary purpose of the position is to plan, organize, develop and direct the overall review for
appropriate admission of residents and patients to the facility in collaboration with the clinical
team and financial team.

•     Previous experience in admissions in a skilled nursing facility or sub-acute setting is
•     Must have a proven track record of integrity and performance

•     Must exhibit sound judgment making skills, patience and compassion
•     Flexibility and multi-tasking skills
•     Embraces a brisk pace in response time in the evaluation and review of potential patients and
residents to the facility
•     Strong analytic and problem solving skills
•     Be capable of maintaining effective working relationships with residents, patients,
residents, family members, staff members and stakeholders
•     Possess the flexibility to approach skilled nursing and long term care services through a
hospitality based model
•     Good computer skills

Essential Functions and Responsibilities of the Position:

•     Responsible for meeting census and admission goals developed by the Administrator and
admissions team
•     Responsible for obtaining referral information to include clinical and financial information
•     Responsible for collaborating with the clinical and financial teams to obtain approval for
new admissions
•     Responsible for conducting facility tours for potential residents and families
•     Responsible for scheduling transport for admission when appropriate
•     Responsible for conducting the admission process of signing in and explaining the admission
agreement and policies of the facility
•     Ensure that all paperwork is complete upon admission
•     Assists with the smooth transition for new residents and families
